Bacterial inflammation plays a major role in many of the painful and frustrating changes women experience from perimenopause through post-menopause. Joint and inflammatory pain may only become noticeable now or in recent years, but the underlying process often began earlier, while oestrogen was still providing protection during menstruating years.
It appears in blood tests and body symptoms like: anxiety, weight gain, bloating, rising TSH, higher LDL, elevated triglycerides, fatigue, hair loss, and insomnia. Women often push through these without connecting them. Meanwhile, oxidative stress builds, increasing the inflammatory load.
How Hormonal Changes Unfold Over Decades
- 30–40 years: Testosterone drops 50%, subtle metabolic shifts begin.
- Late 30s to late 40s: Progesterone drops sharply. High, fluctuating oestrogen met with low progesterone can worsen heavy bleeding, adenomyosis, endometriosis, fibroids, PCOS, high histamine, mast cell issues, allergies, eczema, rosacea, and food reactions.
- 50–57 years: Oestrogen drops sharply. Many women then experience joint pain, frozen shoulder, bursitis, brain fog, insomnia, insulin resistance, and central weight gain.
Oestrogen continues declining: 50% from ages 50–60, another 40–50% from 60–70, and again another 40–50% further from 70–80. This is why it hits some women early in their 50s, and others in their 60s or 70s.
The Bacteria Connection to Changing Hormones
When bacterial imbalance is present in the gut, we often see metabolic issues (brain fog, hair loss, stubborn weight) and/or inflammation (joint pain, muscle aches, bursitis) at the same time as constipation, IBS, heartburn/reflux, bloating, or food intolerances.
Oestrogen spent decades handling protective roles: suppressing inflammatory cytokines, balancing oxidative stress, improving insulin sensitivity, and supporting gut motility. When oestrogen drops, those protective mechanisms significantly decrease.
If the gut microbiome has been affected and oestrogen has gone down, these gut symptoms get bigger. Your body goes into “Fix It Mode,” creating more protective fat tissue to produce more oestrogen as a reaction to the gut inflammation. If bacterial imbalance or gut permeability (leaky gut) isn’t addressed, the cycle continues.
Signs of Bacterial Inflammation at Different Life Stages
In some women, bacterial inflammation appears in their 40s as recurrent UTIs, cyclical migraines, heavy bleeding with poor iron absorption, low micronutrients despite a good diet, frequent infections, and histamine issues.
After 50, it’s often harder to resolve. Joint/muscle pain, stubborn weight gain, and sugar cravings are combined with constipation, bloating, IBS, reflux, and food sensitivities. You start to see pathology changes like rising HbA1c, rising triglycerides, or rising LDL.
The Staphylococcus & Streptococcus Connection
There’s a correlation in bacteria like Staphylococcus and Streptococcus (key players in septic arthritis) that appear at high levels in women with histories of hay fever, psoriasis, eczema, rosacea, endometriosis, adenomyosis, and Epstein-Barr Virus (EBV).
Real Patient Case Studies
Carla, 43 – Adenomyosis & Gastritis
Symptoms: Anxiety, brain fog, trouble maintaining weight, flu-like symptoms with periods, severe migraines.
Pathology: HbA1c 5.7, Homocysteine over 9, EBV Positive (reactivated), slightly elevated ESR, low vitamin D, low B12. From a standard GP perspective, everything looked “normal.” But together, these markers told a different story: chronic low-grade bacterial inflammation, nutrient depletion, and immune system under strain.
Deborah, 58 – Bursitis & Joint Pain Since Menopause
Symptoms: Bursitis (hips, shoulder), joint stiffness, stubborn belly fat, severe brain fog, bloating.
Pathology: CRP at 4.2, TSH at 3.8, elevated LDL, HbA1c at 5.9, low ferritin. Testing revealed bacterial overgrowth and elevated inflammatory markers. Once we addressed the dysbiosis and supported her methylation pathways, her joint pain reduced by 80% in 4 months.
Linda, 67 – Autoimmune Flare-Up After Years of Stability
Symptoms: Rheumatoid arthritis flare, rapid weight gain, insomnia, new food sensitivities, bloating.
Pathology: ESR elevated, ferritin low, vitamin D at 38, rising HbA1c. Linda’s case showed the second wave of oestrogen decline (60–70 age window) unmasking previously managed autoimmunity.
Where to Look & What You Can Do
Between 50–70, as oestrogen continues to drop, symptoms and changes in pathology happen because of genetic vulnerabilities, past infections (EBV, parasites), nutrient deficiencies, methylation issues (MTHFR, DAO/HNMT), gut dysbiosis, and adrenal strain.
